How Does IVF Treatment Progress?

After a detailed infertility evaluation, couples are directed to the most appropriate treatment option based on their specific condition. Depending on medical findings, treatment generally proceeds with one of two main approaches: IUI (Intrauterine Insemination) or IVF (In Vitro Fertilization) / ICSI (Microinjection).

1. IUI – Intrauterine Insemination

IUI is a simpler fertility treatment performed under certain medical conditions.

When is IUI suitable?

  • The woman’s fallopian tubes must be open

  • The man’s sperm count and motility must be within acceptable limits

  • No severe male factor infertility

How does the process work?

  • On the 2nd or 3rd day of menstruation, ultrasound evaluation is performed.

  • If no large cysts are detected, ovarian stimulation begins using oral medication or daily injections.

  • Around the 5th day of treatment, follicle development is monitored via ultrasound.

  • The goal is to develop 1–2 mature eggs.

  • When follicles reach approximately 18–20 mm, a trigger injection (commonly called the “cracking shot”) is administered.

  • 36 hours later, insemination is performed.

During insemination, the sperm sample is processed in the laboratory to select the healthiest and most motile sperm. These are then placed directly into the uterus.

A pregnancy test is performed approximately 15 days after the procedure.

2. IVF and ICSI (Microinjection)

IVF and ICSI are advanced assisted reproductive techniques used when natural conception or IUI is unlikely to succeed.

Conventional IVF

In standard IVF:

  • Eggs and sperm are combined in the laboratory.

  • Fertilization occurs naturally.

  • It is generally preferred when no significant male factor is present.

  • Common female-related causes include:

    • Blocked fallopian tubes

    • Ovulation disorders

    • Endometriosis

ICSI (Microinjection)

Introduced in 1992, ICSI revolutionized fertility treatment, particularly for male infertility. In this method:

  • A single sperm cell is directly injected into each mature egg.

  • It is used in cases of:

    • Very low sperm count

    • Poor sperm motility

    • Abnormal sperm morphology

    • Previous IVF failure

If no sperm is found in the semen sample, surgical sperm retrieval techniques such as TESE, TESA, PESA, or MESA can be performed.

IVF Treatment Flow Step by Step

1. Ovarian Suppression and Stimulation

Treatment begins with medications to regulate and suppress the ovaries. Depending on the patient’s age and hormonal profile, this may start on the 2nd or 21st day of the menstrual cycle.

After suppression:

  • Hormone injections stimulate the ovaries.

  • Unlike IUI, the goal here is to develop multiple high-quality eggs.

  • Ultrasound and blood tests are performed every 2–5 days to monitor progress.

2. Trigger Injection and Egg Retrieval

When follicles reach the desired size:

  • A trigger injection is administered.

  • 36 hours later, eggs are collected under vaginal ultrasound guidance.

  • On the same day, a sperm sample is obtained.

If needed, surgical sperm retrieval is performed on the same day.

3. Fertilization and Embryo Development

In the laboratory:

  • Eggs and sperm are prepared in special culture media.

  • Fertilization is assessed the following day.

  • Embryos are cultured for 3–5 days in controlled incubators.

4. Embryo Transfer

Selected embryos are transferred into the uterus under ultrasound guidance. The procedure is short and typically painless.

After transfer:

  • Hormonal medications are given to support implantation.

  • Blood tests are performed on days 8, 10, and 12.

  • A definitive pregnancy test is usually done 12 days after embryo transfer.
